Abstract

            The development of a care management model for the elderly the Sangkharach Model is a participatory operation of a multidisciplinary team and related network partners To promote the health and quality of life of the elderly The objectives are 1) to create and test the effectiveness of the recession monitoring process for the elderly in the community By a multidisciplinary team 2) To develop a program to educate the elderly by applying the theory of self-efficacy For the elderly who are in decline 3) To measure the effectiveness of using the program to educate the elderly by applying the theory of self-efficacy For seniors who are in recession The research was divided into 3 phases Phase 1 studied the situation problems obstacles, and needs of the elderly in recession The experimental group consisted of 315 people who were patients Phase 2 developed an innovative model for managing care for the elderly and developed a model Program for educating the elderly by applying the theory of self-efficacy for the elderly who are in recession and Phase 3 evaluates the use of innovative models of care management for the elderly The population studied is elderly people with dementia who come to receive services at Elderly clinic of Somdet Phra Sangkharach Hospital 17 Suphanburi Province Specifically selected 166 people at risk for falls It is an experimental group Research tools include questionnaires focus groups Program for educating the elderly by applying the theory of self-efficacy For seniors who are in recession and the Elderly Recession Screening Record Form Statistics used for descriptive data analysis and t-test Group chat program for educating the elderly by applying the theory of self-efficacy For seniors who are in recession and the Elderly Recession Screening Record Form Statistics used for descriptive data analysis and t-test.
